Medicare Part B does exclude certain physician services. These include eye exams and glasses, hearing exams, routine physical exams, routing foot care and most cosmetic surgery. For each 12 month period that you are eligible to sign up for Part B that you do not sign up for it the premium amount increases by 10%.
